Membership Dues for heart and soul pediatrics

The membership dues listed below will be valid for the entire first year of membership. Our membership is limited. Once we are full, we will not accept any more patients and will open a waiting list for enrollment. 

There is an initiation fee of $150 per family to enroll as a new member of Heart and Soul Pediatrics.

• $250.00 per month for 0-60 months (5 years)
• $150.00 per month for 6 years to 21 years
• Maximum per family $500.00

A $50.00 per month discount will be given to those families with two or more children under 5 years of age.

A membership can be cancelled at any time with 30 days’ notice.
