Dr. Colón and Hillary Cash, NP are happy to welcome new families to Heart and Soul Pediatrics. We are a membership-based pediatric practice in Marietta, Georgia, providing personalized care for children, adolescents, and young adults.

Heart and Soul Pediatrics is a membership-based practice designed to provide families with more time, greater access, and a more personal relationship with our team.

Membership includes benefits such as longer appointments, easier access to sick visits, telehealth when appropriate, direct communication through the patient portal, priority scheduling, and greater continuity of care.

Complete the membership inquiry form below to get started. We will contact you to learn more about your family, answer questions about membership, and explain the next steps.

Heart and Soul Pediatrics provides preventive care in accordance with current evidence-based pediatric standards and recommendations.

Please complete the Heart and Soul Pediatrics Intake Form for each new patient joining the practice.

After we receive the form, we will contact you with any additional authorizations or records-release documents that are needed.

All new patients must complete the required authorization forms for Heart and Soul Pediatrics to obtain medical records from your child’s current or previous healthcare providers, as well as authorization identifying individuals who may receive protected health information.

Forms may be completed electronically or in person at the office.

Our office will contact you to schedule the first appointment once the membership contract has been signed, all required forms have been completed, and the necessary medical records have been received and reviewed by Dr. Colón.
